Test: Prone Gapping Test - Sacroiliac Test (Hibb's Test)
Video Link
Background/Purpose
Although this tests lacks sensitivity and specificity in isolation, it does provide a better predicting value in combination with the other tests for the SI joint. Because this test involves using the hip as a lever to act on the SI joint, pain or guarding may result if there is involvement of the hip joint or the surrounds muscles. This may involve specifically the hip external rotators which will likely be stretched during this maneuver. It is important to use additional tests to isolate the various structures with a failing test.
Procedure
With the subject prone, the clinician flexes the knee of the testing side to 90 degrees. The clinician then internally rotates the thigh by pushing the flexed leg away from mid-line. This action will likely create some posterior gapping in the SI joint. The amount of motion achieved should be compared to the contralateral side. Also note if this reproduces the subject's pain/complaint.
Performed: Unilaterally
