Test: Thigh Thrust (Posterior Pelvic Provocation) Test
Video Link
Background/Purpose
This test is known by multiple names including Thigh Thrust, Posterior Shear Test, Posterior Pelvic Provocation Test.
The purpose of this test is to reproduce the patients sacroiliac joint symptoms.
In some cases the test will reduce symptoms and this could possibly by due to a repositioning of a posteriorly rotated ilium anteriorly.
Procedure
With subject supine, the subjects leg is positioned so the hip is flexed to 90 degrees and possibly slightly adducted. The clinician applies a posteriorly directed force through the femur. Some variations also include the clinicians opposite hand cupping the sacrum which creates additional shear on the SIJ. A failed test is indicated if the subject's pain is reproduced. It should also be considered that a palliative response could also indicate that SIJ dysfunction is present.
Performed: Unilaterally
